Job Responsibilities:

Will offer clubhouse members a single point of contact with the program to ensure access and a coordinated response to tangible member needs such as: food, shelter, clothing, transportation, education, etc. The Case Manager duties are similar to community support interventions this includes but is not limited to: home/residence visits to provide motivational support, follow-ups on absenteeism, care coordination for additional services, facilitation of appointment keeping for wraparound services, telephone calls providing appointment reminders and barrier checks, conflict resolution around service related issues and other compliance related issues. Services may be provided to members in group or individual settings. Case manager provides on- going support for youth and families in after-care services. It may be helpful to have more than one case manager in order to ensure case coverage.


Position Responsibilities:

Monday-Friday 9:30-6:30. May assist with activities with youth on weekends as needed (to be flexed during week)



Competencies:

1. Knowledge of diagnostic impressions or criteria. 2. Knowledge of how client advocacy system operates 3.  Knowledge of crisis intervention techniques 4. Knowledge of individual and group therapy techniques. 5. Knowledge of various therapeutic models and intervention as directly related to area  of assignment. 6. Clinical skills (Advanced)



 
Physical Demands
 
General office environment.  Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Employee must occasionally lift and/or move up to 25 pounds.  Must have ability to sit for long periods of time at a computer.  Lighting and temperature are adequate, and there are no hazardous or unpleasant conditions caused by noise, dust, etc. Employee must be able to travel between Unison sites.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Required Minimum Qualifications:

HS Diploma with 3-5yrs MH experience: Preferred Bachelors in Social Service related field with at least one year of experience in MH exp.
